Exhibit – Student Petition to Add a District-Funded or Non-Funded Athletic Program

Students signing the form below are indicating that they would participate in the above athletic
activity. There must be at least 25 signatures on the form by students currently in Grades 8 thru 11
for the proposed athletic activity to be considered. (Use additional forms if necessary)
If the activity were approved, each student signing below would:
Attend each practice scheduled before/after school and on weekends.
Attend competitions, which may be after school and on weekends.
Quit any club/team which has a conflict with the proposed athletic activity in the same season.

This form must be submitted to the Assistant Principal for Athletics at least ninety (90) days before the first
scheduled practice of the proposed initial season, or, in the case of fall sports, by May 1 of the
preceding school year.
