Exhibit – Authorization for Electronic Network Access
CONSENT AND RELEASE FORM
Teacher/Staff Authorization:
- I understand and will abide by the Authorization for Electronic Network Access.
- I understand that the District and/or its agents may access and monitor at any time without prior notice the use of any and all of their equipment no matter where the device(s) are located.
- My use of the device(s), Internet use, e-mail services and any downloaded materials or media, any items contained on or created using any of Glenbard's electronic devices including laptops, cell phones, or any similar devices can be monitored and reviewed at anytime.
- The monitoring of devices can/may include the use of system log files screen captures, key captures, images captured by built-in camera or other connected devices and/or any other method that becomes available to ensure the device is operating within Board policy, to protect the District's assets or inventory control.
- I understand that any device(s) loaned to me during my employment are solely for my use and not to be used by family members or anyone else for any reason at any time.
- I understand that I am responsible for the replacement cost of any devices and equipment that are lost, stolen,or damaged while on loan to me.
- I will not attempt to bypass restrictions or attempt to disable/compromise any security/monitoring functions.
- I further understand that should I commit any violation, my access privileges may be revoked and school disciplinary action, up to and including termination, and/or appropriate legal action may be taken.
- In consideration for using the District's electronic network connection and having access to public networks, I hereby release the District and its Board members, employees, and agents from any claims and damages arising from my use of, or inability to use the Internet.
- As the sponsoring teacher, I do agree to instruct the student(s) on acceptable use of the network and proper network etiquette. I recognize my responsibility for supervising student(s) use of the electronic network. I understand that my student(s) may use the network for individual work or in the context of another class, and that I cannot be held responsible for the student(s) use of the network.
